Registration for Fox Valley Sailing School : Youth/Teen Level 1

This class is intended for beginning sailing students, aged 7-18. Instructors balance camp activities based on student maturity, confidence and demonstrated sailing acumen. Students will sail on Optimist Prams (Optis) and Club 420s, with some access to Lasers depending on student skill level.

Note to registrants, a US SAILING Member ID is not necessary.

General Liability Release and Waiver of Claim

In consideration of the acceptance applicant as a student (Student) in the Fox Valley Sailing School (the School), Student, or parent/guardian of Student, by signing this waiver form hereof agrees that the School, sponsors or supporters of the School, the City of Neenah, the State of Wisconsin, and any and all other persons or entities involved in the School, together with all their agents, sub-contractors, employees or volunteers SHALL NOT BE LIABLE for the death or injury to Student, for any loss due to theft of or damage to the property of Student, or for any other consequential or incidental damages in any manner whatsoever where any such liability is attributable to the absence of ordinary or even slight care by the School in the conduct of the School activities.

Student, or parent/guardian of Student, further agrees not to bring or cause to be brought any suit on any such claim or course of action and acknowledges that the releases and waivers referred to herein shall apply regardless of whether liability is asserted on the basis on contract, strict liability or ordinary gross negligence.

Student, or parent/guardian of Student, further acknowledges that the execution of this Release and Waiver is continuing in nature and that it is his/her free and voluntary act.

Furthermore, Student or parent/guardian of Student does hereby grant full permission to the School to use any photographs, videotapes, motion pictures, recordings or other records of the activities of the School and to do so without compensation to Student or parent/guardian of Student. This Release and Waiver shall be construed under the laws of the State of Wisconsin. SIGNATURE BY STUDENT, OR PARENT/GUARDIAN OF STUDENT, VERIFIES THAT HE/SHE READ AND FULLY UNDERSTANDS THE FOREGOING and further certifies that the information provided by Student or parent/guardian of Student on his/her entry form is true and complete.
